1. In this Privacy Policy, 'us' 'we' or 'our' means Follicle Pty Ltd (ACN 681 921 237) and our related bodies corporate. Our Privacy Policy sets out how we collect, use, store and disclose your personal information.

5. We may also collect information from or in relation to your mobile, computer or other device (Device) when you visit our website. The information we collect may also include (but is not limited to) your Device type and operating system, IP address, Device ID, browser type and language, geographic location when using the website, access times, and usage information.
6. If at any time you provide information or data of a third party to us, you warrant and represent that:
6.1. you have already given written notice to the third party of the intention to use the information or data for the purposes and in the manner set out in the Privacy Policy from time to time;

8.1. liaise with our Clinic, fill out patient forms or attend consultations or treatments;

8.3. submit an enquiry or request a quote for our services;

8.5. invest in our business or enquire as to a potential purchase in our business; and/or
8.6. engage us to perform our services and procedures.

In addition, if you apply for a job or position with us we may collect certain information from you (including your name, contact details, working history and relevant records checks) from any recruitment consultant, your previous employers and others who may be able to provide information to us to assist in our decision on whether or not to make you an offer of employment or engage you under a contract. This Privacy Policy does not apply to acts and practices in relation to employee records of our current and former employees, which are exempt from the Privacy Act.

10.2. to develop and manage your treatment plan, including any pre-procedure and post-procedure care;

12.2. other healthcare professionals involved in your treatment;
